Fee Waiver Request Form 2027-28 Application Cycle

Please complete this form along with any supporting documentation prior to the deadline stated before.

Application fee waivers are available for applicants to the following graduate programs who meet additional eligibility criteria:

*Current ScM Students who plan to apply for the PhD program for Fall 2027 should follow instructions sent by the program at the beginning of the year. Admitted applicants will be given a fee waiver upon admission.

Eligibility Criteria:

Applicants may only apply for 1 fee waiver per application cycle and they are only available only to cover the primary application fee within the SOPHAS system; secondary application fee waivers are not available.

Applcation Fee Waivers are available for U.S. Citizens and Permanent Residents who meet one of the eligibility requirements listed below.

Applicants are eligible for an application fee waiver for:

Documented financial hardship

Financial hardship is defined as earning less 225% the poverty level based on your household size or Pell Grant Eligibility. Documentation may include a financial award letter from the college you currently attend or a copy of FAFSA or Student Aid Report (SAR). Please redact all social security numbers and individual taxpayer identification numbers from the documentation before submitting

Documentation from one of the following:

U.S. Military service

Eligibility for Vivien Thomas Scholars Initiative (VTS) and application ready for submission to a BSPH VTSI-eligible PhD program

Current enrollment as a Johns Hopkins University undergraduate student

You must meet eligibility requirements for VTSI, which can be found here: https://provost.jhu.edu/about/vivien-thomas-scholars-initiative/admissions/#howtoapply Please upload an unofficial transcript from your VTSI Eligible School.

To be eligible to apply to the VTSI, one must have attended a VTSI-eligible institution after completion of high school and attendance must have been for undergraduate study.
